Medical Summary DVM Intake Exam Estimated age: 3yrs Microchip noted on Intake? neg History : Subjective: Observed Behavior – allows all handling, but didn’t allow temp to be taken Evidence of Cruelty seen – no Evidence of Trauma seen – yes Objective P = 180 R = 24 BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: pink gums, mild dental tartar and gingivitis, no URI signs PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: both testicles descended MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, approx 8cm full thickness open wound on R side of neck with mild purulent d/c CNS: mentation appropriate – no signs of neurologic abnormalities Assessment wound R side of neck, infection DOH bite victim Plan convenia 80mg/ml: 0.45ml SQ simbadol 1.8mg./ml: 0.6ml SID x3days rec consider sedation for wound cliping, cleaning edges, attempting to close wound +/- drain: could be performed at same time as neuter Prognosis: good with appropriate management SURGERY: Okay for surgery
