Medical Summary DVM Intake Exam Estimated age: 2-3 years Microchip noted on Intake? neg History : brought in as a stray; had vomitus in carrier that consisted of live roundworms and mucous; had slight appetite with canned food Subjective: BAR Observed Behavior – NICE BOY!!! easy to handle Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T = not taken P = WNL R =WNL BCS 5/9 EENT: OS-severe chemosis, purulent discharge; severe blepharospasm Oral Exam: mm pk, moist; CRT <2 sec; minimal tartar; mild discoloration on canines PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, slightly dull haircoat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal:not performed Assessment 1. Conjunctivitis OS 2. roundworms Plan deworm now and again in 2 weeks cerenia SQ once erythromycin OU BID x 10 days Prognosis: excellent SURGERY: Okay for surgery
