She has a voracious appetite and is friendly and attention – seeking. VIXEY is very thin and may have some underlying issues which will need some follow up vet evaluation. DVM Intake Exam Estimated age: ~8yrs Microchip noted on Intake? no History : Owner surrender – only had cat for 3 weeks, found outside Subjective: BAR. ~5% dehydration. Voracious appetite. Normal urine, large pile of soft stool Observed Behavior – Allows all handling, attention seeking Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = WNL R = WNL BCS 3/9 EENT: Eyes clear, moderate waxy debris, no nasal discharge noted, icteric sclera and pinnae Oral Exam: Mild dental tartar/gingivitis, icteric mm PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated, distended cranial abdomen, doughy U/G: Female MSI: Ambulatory x 4, skin free of parasites, no masses noted, dirty, unkempt hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Senior pet, icteric, underweight, dehydrated Plan: Scheduled CBC/Chem/T4/lytes/UA, consider abdominal radiographs. Rec LRS 50 SQ x 3 days. Prognosis: Fair, pending diagnostics SURGERY: Permanent waiver due to age, probable liver disease

[Spay/Neuter Waiver – Age] It is the policy of ACC not to perform surgery on any animal over the age of 8-10 years due to the higher risks incurred in a shelter setting. The veterinarian is hereby issuing a permanent spay/neuter waiver, from the spay/neuter requirements of the City of NY due to the estimated age of this animal. ACC does recommend you consult with your veterinarian to determine if surgical sterilization is appropriate.

Monitor condition – underweight, icteric, senior cat S/O: Q/BAR-H (SQF still present). Allows handling. Excellent appetite since in shelter. 2 piles of regurgitated dry food in cage. Normal urine and stool in litter. EENT: Icteric sclera and mm, mild to moderate dental tartar/gingivitis, no ocular or nasal discharge HL: Normal thoracic auscultation ABD: Slightly distended cranial abdomen, soft, non tender INTEG: Unkempt MS: Ambulatory x 4 UG: Female CBC Moderate regenerative anemia Moderate to marked leukocytosis characterized by a moderate neutrophilia, mild monocytosis Moderate thrombocytopenia All other results WNL Chem Hyperproteinemia and hyperglobulinemia Mildly increased liver enzymes – ALT, ALP, GGT, Tbili All other results including T4 WNL A: Red blood cell destruction suspected with regenerative anemia +/- mild liver involvement- infectious vs autoimmune vs other Dental disease Inflammatory leukogram Regenerative anemia P: Adding doxycycline 0.6ml PO q24 x 10 days for possible infectious causes. May need course of steroids if auto-immune disease. Fair prognosis

ENRICHMENT NOTES:: 01/04/18 Resting by the front, calm and relaxed. Meows softly and lies there, soft body posture. She accepts petting and begins to purr, leaning in and stretching during interaction. Doing well!
