Medical Summary DVM Intake Exam Estimated age: 2 years Microchip noted on Intake? negative History : Brought in over the counter from a feral cat colony Subjective: stray Observed Behavior – did not allow any handling without sedation Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective : BARH, sedated exam P = 240 bpm R = 24 bpm BCS: 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: no dental disease PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MI, 2 testicles descended MSI: Ambulatory x 4 before sedation, fleas noted, no masses noted, healthy hair coat, abrasions on face likely from feral trap CNS: mentation appropriate – no signs of neurologic abnormalities Assessment Abrasions on face likely from feral trap Fleas Otherwise healthy Plan: Telazol 0.1ml given IM for exam Needs behavioral eval Prognosis: excellent SURGERY: Okay for surgery
