Medical Summary DVM Intake Exam Estimated age: 5 weeks Microchip noted on Intake? negative History : stray Subjective: BAR Observed Behavior – very sweet. Easily handleable and easy to pick up. Enjoys being held and did well for all medical handling and procedures Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = eupneic BCS 5/9 EENT: Purulent ocular d/c. Purulent nasal d/c. Oral Exam: kitten dentition. No oral ulcers seen. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic. No audible congestion ABD: Non painful, no masses palpated U/G: MI MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt haircoat and food and crust around face CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: URI Underage Plan: Doxycycline 10mg/kg PO SID x10d Erythromycin OU BID x10d Recheck in 5, 10 days Prognosis: Fair SURGERY: Temporary waiver for surgery due to URI
