DVM Intake Exam Estimated age: 2-3 Microchip noted on Intake? no History : stray Subjective: BAR, hydrated Observed Behavior – A little stiff on exam and treatment, but allows full workup/handling with minimal restraint. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = 120 R = 20 BCS: 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: fractured 104 and 204, teeth in good condition otherwise PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact, both testes present MSI: Ambulatory x 4, skin free of parasites, scattered areas of alopecia/self grooming along dorsum and dorsal pelvic region, no ectoparasites noted, could be healing bites/scratches vs FAD CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: minor resolving dermatitis in apparent good health otherwise. Plan vaccinate deworm flea treat combo test microchip Prognosis: excellent SURGERY: Okay for surgery
