Medical Summary DVM Intake Exam Estimated age: 4-5 weeks Microchip noted on Intake? no History : Subjective: BAR Hydr wnl very good appetite Observed Behavior – very easy to work with, even took temp w/out complaints Evidence of Cruelty seen – None observed Evidence of Trauma seen – None observed Objective T = 100.1 P = 196 R = wnl BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: nsf PLN: No enlargements noted H/L: NSR, 1/6 hm CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated distended U/G: MI nsf MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment:: distended abd r/o parasites, malnutritonother Plan: n tx indicated at this time Prognosis: good SURGERY: Temporary waiver due to underage and underwgt
