History : Stray kitten with conjunctivitis
Subjective: Quiet, slow to respond to stimulation and but once pt begins to respond, he mews and moves around
Observed Behavior – Allows all handling
Objective
QAR, estimated 8-10% dehydrated, BCS 2/9
EENT: Eyes crusted shut – once opened, copious yellow purulent discharge is expressed. AU moderate black granular discharge. Muzzle is covered with dried food debris. Possible crusty nasal discharge although it could be just food debris in the nose.
Oral Exam: Clean deciduous teeth
PLN: No enlargements noted
H/L: NSR, NMA, Lungs clear, eupnic
ABD: Soft, non painful, no masses palpated
U/G: Male, testicles palpable
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Normal externally
Pt defecated after the exam – large volume of very smelly, soft, formed, feces
Assessment:
1. Likely ear mites
2. Severe conjunctivitis
3. Likely URI
4. Emaciated
5. Dehydrated
Prognosis: Fair – if pt responds well to the next 24 hours worth of treatment, pt should do well
Plan:
1. LRS 20 ml SQ q12 x3 doses
2. Clean eyes, terramycin OU BID x10 days
3. Clavamox 0.1 ml PO BID x10 days
4. Tresaderm AU BID x10 days – clean ears q3 days
5. Deworm on regular q 2 week schedule
