*** SAFE 10/26/17 ***TOMMY was being fed by someone when he disappeared and then returned sick. He has some kind of wound on the tongue and is drooling and holding mouth open due to pain. Needs further medical and a new home.

I came into the shelter as a stray on 17-Oct-2017.
Tommy 10026 is at risk for a severe lingual wound. Behaviorally, he has been very solicitous of attention and is very affectionate. He is suitable for a first-time adopter.

History : was being fed by family for a while; he disappeared for a few days and he seemed sick when he returned, was vomiting and had green liquid coming from mouth Subjective:QAR Observed Behavior -docile, allows handling Evidence of Cruelty seen -n Evidence of Trauma seen -n Objective BCS 5/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam:mm pk, sl tacky with hypersalivation; CRT <2 sec; severe swelling and necrotizing wound along L side of tongue with foul odor and green discharge; holding mouth open due to pain PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal:grossly normal Assessment Severe lingual wound Prognosis:open Plan: Place IVC, run LRS at 15 ml/hr Cerenia 0.4 ml IV SID x 3 days Onsior 0.4 ml SQ SID x 3 days Clindamycin SURGERY: okay for sx
Presented with severe, necrotizing wound along L side of tongue; started on IV LRS @ 15 ml/hr, cerenia, clindamycin, onsior and simbadol S/O -no obvious appetite -QAR, docile and very sweet; allows all handling -mm pk, CRt <2 sec; moderate hypersalivation, initially was very blood tinged but seemed to improve through the day -severe wound along entire L side of tongue; tissue appears dark blue/black along dorsal aspect and slightly green along glottis; moderate swelling but appears improved from yesterday -good movement of tongue, able to retract -mild to moderate pharyngeal congestion due to tongue swelling and drainage -eupnic, heart/lungs WNL -soft, nonpainful abdomen A Lingual wound, severe, necrotizing P Decreased IVF to 12 ml/hr Continue cerenia, onsior, clindamycin and simbadol Add baytril 22.7 mg/ml: 1.7 ml IV SID If no appetite by tomorrow, start syringe feeding prognosis: fair, suspect that he will lose a lot of tongue tissue to necrosis but appears to be improving and could likely due well long term

Auscultation wnl noted patient is BARH, eating well A: lingual ulcerations r/o herpes, calici P: CWSC, dc IVF therapy
S/O -BAR, very affectionate and interactive great appetite!!! -mm pk, moist; CRT <2 sec -severe wound along L side of tongue looks significantly improved!!! all swelling resolved, no areas of concern for necrosis; moderate residual erythema, mild ulceration -eupnic, heart/lungs WNL -soft abdomen -slightly underweight A lingual wound-resolving!!! P Continue clindamycin, simbadol Last day of injectable baytril, switch to oral tomorrow prognosis: excellent!

Upon intake Tommy did not allow any handling. He was growling, hissing and swatting at the carrier. Due to possible injury in the ear he was not handled to reduce stress.

MEDICAL BEHAVIOR:: Observed Behavior -docile, allows handling
ENRICHMENT NOTES:: Recent history of severe vomiting. Curled up in litter box, resting, soft eyes and body. Lay in place when door opened, leaned into pets, purred, rolled over, closed eyes. Very sweet, clearly not feeling well.
