Medical Summary DVM Intake Exam Estimated age: 2yrs Microchip noted on Intake? yes 981020019757307 History : Originally surrendered by the police as a stray. Was adopted and then surrendered again due to the owner being pregnant and her son having allergies. Subjective: Observed Behavior – relaxed in cage, eating, allows all handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = 120 R = 30 BCS 3/5 (5/9) EENT: Eyes clear, ears mild ceruminous debris, no nasal discharge noted Oral Exam: Healthy dentition PLN: No enlargements noted H/L: Difficult to auscult over purring, but no obvious murmurs or arrhythmias ABD: Soft, but cries on palpation, no palpable masses; BM in litter box was light brown, mostly formed with specks of frank blood U/G: recently neutered, site appears to be healing well MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment – Apparently healthy Blood in stool DDx: Intestinal parasites, IBD, Open Plan – monitor for blood in stool Prognosis: Good-Excellent SURGERY: Already neutered
