Tom 8871 – 4 yr. male chocolate point Siamese
Lying in back of kennel with a low body, head facing kennel wall. He tolerates petting along his head and body while remaining very still, body tense. Needs more time to adjust. Sprayed feliway.

Oral Exam: grade 2 periodontal disease
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: male intact
MSI: Ambulatory x 4, fleas, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of
Assessment: in apparent good health
recommend periodontal therapy
continue to monitor at ACC
SURGERY: okay for surgery
