I came in the shelter as a FOSTER on 07/01/2017 from NY 11385, owner surrender reason stated was FOSTERSURG. I came in with Group/Litter #K17-098344.

Exam Type BEHAVIOR
KNOWN HISTORY: Soda Pop was brought in as a stray, so there is no information on his behavior in his previous home. He allowed all handling during intake. MEDICAL BEHAVIOR: Observed Behavior – Very social, active and friendly. Allows all handling. ENRICHMENT NOTES: 05/25/17 Lying curled up in back of kennel with sibling. Allows petting along head and body while remaining relaxed and smelling my hand. At front of kennel by the end of the interaction, vocalizing and reaching paws out through kennel door. Doing well! EVALUATION: Cage Condition: Cage is re-arranged Reaction to assessor: Soda Pop was playing with his siblings upon approach. Reaction when softly spoken to: Soda Pop pauses and looks at the assessor with soft eyes. Reaction to cage door opening: Soda Pop remains soft and relaxed. Reaction to touch: Soda Pop sniffs the assessor’s hand and allows petting. He begins to purr and gently plays with the assessor’s hand by patting it with his paw. Reaction to being picked up: Soda Pop remains soft and continues to purr when held. ACTIVITY LEVEL: Moderate, Lively VOCAL: Quiet CHARACTER TYPE: Social, Sweet, Playful BEHAVIOR SUMMARY: Beginner Soda Pop interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. Subjective: Observed Behavior – Very social, active and friendly. Allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=lt pink, moist, CRT<2 s. HR>240, RR=24. BCS=5/9. ORAL: All deciduous teeth erupted, no oral lesions. EENT: No ocular discharge or chemosis. No nasal discharge or sneezing PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias, no dyspnea. ABD: Non painful, no masses/organomegaly palpated. U/G: Male. MSI: Ambulatory x 4, no lameness. Full, clean haircoat. No fleas or skin lesions. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Approx. 4 week old kitten, apparently healthy. P: Offered food – excellent appetite. Excellent prognosis. Rec. placement. Temporary waiver from neuter surgery due to young age.
