Medical Summary DVM Intake Exam Findings History: Owner surrender due to personal problems Subjective Observed Behavior – seems nervous, but allows all handling Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective T = NA P = 190 R = 60 BCS 3/5 (5/9) EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Healthy adult dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: both testicles palpable scrotally MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment – apparently healthy Medical Behavior Status – GREEN (does not solicit attention (also very nervous), but allows all handling with no signs of aggression) Medical Status – 1 Asilomar Status – HEALTHY Fast Track?
