I came in the shelter as a STRAY on 08/05/2017 from NY 10035, owner surrender reason stated was TNR. I came in with Group/Litter #K17-106843.

ACTIVITY LEVEL: Steady VOCAL: Quiet CHARACTER TYPE: Skittish MEDICAL BEHAVIOR: 8/5/17- Observed Behavior – will flee, exam through net – did not aggressively strike or hiss EVALUATION: Cage Condition: No change Reaction to assessor: Tigerlily remains tense, tightly curled up against the back of the cage. Reaction to cage door opening: Tigerlily remains stiff, distance increases, alert with eyes dilated. Reaction to touch: Tigerlily does not allow the touch and attempts to escape the cage. BEHAVIOR SUMMARY: Tigerlily is displaying behaviors that preclude placement in the adoptions room and/or may require further investigation before placement in a home. She has tried is extremely fearful in the shelter environment and does not currently tolerate petting or handling. The behavior department feels that placement with a New Hope Partner is the best option at this time

Hx: Presented for pre-neuter exam. Was noted to have slight clear nasal discharge on intake exam. S: Due to behavior, visual exam was performed with pt in feral den. Pt sat in one place and watched my movements. O: BAR, MMs pink EENT: Mild mucoid ocular discharge OU. Mild mucoid crusts at the nose. H/L: Eupnic, not ausculted M/S/I: No lesions noted on visual exam Neuro: Alert and appropriate A: Mild URI and conjunctivitis P: Move to isolation, start doxycycline 0.5 ml PO SID x10 days and erythromycin OU BID x10 days 1088
