SAFE 6/20/17 **1-Year Old White Kitty – TC – WITH MULTIPLE FRACTURES NEEDS EMERGENCY VET CARE ASAP!**
TC A1115824 – 1 yr. male white cat, stray:
DVM Intake Exam
Estimated age: 1 year
Microchip noted on Intake? no
History : Stray, picked up by van, not easily handleable
Subjective: QAR, 7% dehydrated, coated in feces and urine in the hind end. sedated for exam with 0.15 telazol im
Observed Behavior – hissing, struggling to escape, ; sedated exam performed (0.15 mL Telazol IM).
Evidence of Cruelty seen – none
Evidence of Trauma seen – yes, possible hit by car
EENT: Moderate dried ocular discharge OU, enophthalmos OS, no nasal discharge noted
Oral Exam: multiple fractured teeth in all arcades. Mandible laterally displaced to the left and overriding the maxilla. Deep laceration of the lip over the right mandible; visible pus present.
PLN: No enlargements noted
H/L: NSR, NMA, Lungs clear, eupnic; no evidence of pneumothorax
U/G: intact male, significant fecal and urine staining.
MSI: Non-ambulatory in the hind prior to sedation but has motor bilaterally. Significant bruising and palpable fracture of the right femur. Radiographs: distal femoral fracture on the right, femoral neck fracture on the left, pelvic symphyseal fracture with mild displacement, pubic physeal fracture on the right, mandibular symphyseal fracture. Some evidence of gas tracking in the right thigh; shaved, no open wound seen.
CNS: mentation appropriate – no signs of neurologic abnormalities. Could not perform full neuro exam – sedated.
Assessment: multiple traumatic fractures requiring surgical repair of both hind limbs and jaw, as well as extensive dental extractions.
Plan: Gave Simbadol 0.33 mL and Onsior 0.25 mL, maintain on stray hold with pain management while New Hope plea. Convenia .22ml
Prognosis: good for return to function with multiple/extensive surgical interventions.
SURGERY: Temporary waiver due to multiple fractures
