Observed Behavior – QARH. Allows all handling.
Evidence of Cruelty seen – No
Evidence of Trauma seen – No
P = WNL R = WNL BCS 6/9
EENT: Eyes clear, Mild conjunctiviits, ears clean, mild mucoid nasal discharge noted
Oral Exam: Moderate dental tartar/gingivitis, mild gingival recession,
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: URI, conjunctivitis, dental disease
Plan: Rec doxycycline 1.3ml PO q24 x 10 days (gave first dose), and erythromycin OU q24 x 10 days
