Medical Summary DVM Intake Exam Estimated age: 3 YO based on size and dentition Microchip noted on Intake? neg on DVM intake/scanned neg History : surrendered Subjective: Timid/nervous. Was okay for initial handling, after exam, starting growling/hissing and bolted under kennels. Was netted for diagonstics and vaccinations/MC placement Objective BCS: 4/9 EENT: Eyes clear, ears brown debris AU with pin point areas of hair loss (did not flourese) no nasal discharge noted Oral Exam: #104 crown fracture, normal color of tooth. teeth otherwise clean, healthy gums PLN: No enlargements noted H/L: NSR, NMA, CRT < 2 Lungs clear, eupnic ABD: tense, non painful, no masses palpated U/G: both testicle descended MSI: Ambulatory x 4, skin free of parasites, no masses noted, medial aspect of left front arm 1cm area of hair loss with central area of lichenified skin (woods positive). Pinnae pin point alopecia CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: grossly normal Assessment: 3 YO MI DSH 1. ringworm 2. fracture #104 3. thin BCS 4. Debris AU r/o mites vs other Plan 1. Collected specimen for DTM (done at DVM intake) 2. woods lamp positive – left front leg; medial aspect 3. clean ears and start RX: tresaderm AU BID x 10 days (did not do at intake d/t behavior) 4. fractured tooth – do not believe causing pain/discomfort; no treatment at this time Prognosis: good with care SURGERY: Temporary waiver due to ringworm
