Medical Summary DVM Intake Exam Estimated age: young (approx 1yr) Microchip noted on Intake? no History: Stray Subjective: Cat is BARH. Observed Behavior – At front of carrier, seems curious and friendly, sniffs and rubs into hand. When carrier opened cat lifts paw as warning and escalates to swatting and biting assess-a-hand when petted. Vocalizes (hissing and growling) and fights restraint to give Telazol. Gave 0.15 mLs IM and applied artificial tears OU. Sedation was adequate to perform exam and treatments, but cat was still tense and growling throughout. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = 170 R = wnl BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Healthy adult dentition; Grade 0/4 dental dz PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: wnl MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Apparently healthy Plan: Acceptable Community Cat Candidate. Prognosis: Excellent SURGERY: Okay for surgery
