Medical Summary DVM Intake Exam Estimated age: 6 wks old Microchip noted on Intake: Negative History: Stray Subjective: Observed Behavior – BAR Evidence of Cruelty seen – None Evidence of Trauma seen – None Objective: Hydrated; T =NT P =200 R =26 BCS 5/9 EENT: Eyes green dischrage. severe conjuctivitis, no nasal discharge noted Oral Exam: No dental tartar, all deciduous teeth are present, MM Pink and moist, PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Intact female MSI: Ambulatory x 4, skin free of parasites, no masses noted, dirty hair coat covered with lice CNS: Mentation appropriate – no signs of neurologic abnormalities (full neurologic exam not performed) Rectal: Not performed Assessment 1. Underage/undeweight 2. URI 3. No eating 4. Lice Plan/Treatment/Recommendations – Wt:1.3 lb – Vaccines (FVRCP SQ RFL) – Dewormed (Pyrantel and ponazuril) – Flea prevention treatment – Lice treatment – Erythromicin OU BID 7 days – Amoxi-clav 0.3 ml PO BID 10 days Prognosis: Good once is treated SURGERY: No Temporary waiver due to underage/ underweight and URI Rec NH placement
