DVM Intake Exam Estimated age: 3-4 yo Microchip noted on Intake? no History : stray brought in as SNR candidate Subjective: Observed Behavior – sternal in back of kennel, tries to run when handled Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = not done P >200 R = eupnic BCS 2/9 EENT: OS ptosis and mild 3rd eyelid elevation, crusting discharge OU, back debris AU, no nasal dc Oral Exam: missing central max and mand incisors, pink mm PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: liguquid-filled intestines, kidneys subjectively normal in size and shape U/G: intact male, both testis descended MSI: Ambulatory x 4, severe flea infestation, no masses noted, hindlimbs and tail covered with liquid greenish brown diarrhea CNS: dull mentation appropriate – no signs of neurologic abnormalities Rectal: A: FeLV positive gi inx vs gi toxic vs gi FB vs other gi vs non gi P sedated with 0.06ml Telazol IM bathed pelvic rads: CBC Chem IVC, bolus 50ml IVF, then at 8ml/hr Prognosis: guarded SURGERY: Temporary waiver due to severe diarrhea, emaciation
