Medical Summary DVM Intake Exam Estimated age: 4yrs Microchip noted on Intake? negative on scan History : o surrender, h/o declawing, reported to be neutered Subjective: Observed Behavior – VERY FRACTIOUS, growling, swatting, used net to obtain, cannot perform exam- only visual Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective unable to obtain TPR BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: unable to examine PLN: unable to examine H/L: unable to examine ABD: unable to examine U/G: reported to be neutered, not examined MSI: normal fur coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment apparently healthy from visual exam DECLAWED Plan attempt exam again in 4 days, give time to adjust should only be indoors since declawed will need to perform FeLV/FIV snap and MC at another time Prognosis: good
