Medical Summary 01/12/17 14:36 scan neg approx 1 yo MI DLH S: brough in as stray, urinating on himself in kennel, eating O: BAR, dehydrated AU: WNL OU: WNL Oral: no lesions, no dental tartar, deciduous dentition consistent with estimated age Int: abrasions on caudal medial portions of hindlimbs, fur on hindlimbs and tail covered in urine Lnn: WNL CV: NMA, s&s pulses, pale tacky mm, HR 180 Resp: clear lungs, eupnic Abd: severely enlarged firm bladder, urinary incontinence Repro: intact male MS: 4x ambulatory, BCS 2/9 with moderate generalized mm atrophy Neuro: mentation WNL, no ataxia A: partial urethral obstruction vs oliguria/anuria 2nd to AKD vs pelvic trauma vs other P: 80 ml LRS SQ0.25ml Buprenex OTM BID 0.30ml Cerenia SQ 0.15ml Diazepam IV addendum very difficult to express bladder under sedation, able to express some hemorrhagic urine, Sunny is eating and drinking water, posturing in litter box CBC: Hct 22% (in face of mild dehydration), mild neutrophilia (14K) Chem: BUN too high to read, Crt not read, Phos 14.5 (2x hi range), potassium 6 r/o urethral obstruction and post renal azotemia vs congenital kidney dz vs neoplasia vs other NH placement tonight requires IVC, sedation to unblock, EHR if no placement foung by 6pm px: good with appropriate treatments
