DVM Intake Exam Estimated age: ~3-4 years Microchip noted on Intake? negative History : o/s, part of a hoarding case Subjective: BARH Observed Behavior – nervous and will flee if given the chance. Easily handleable. Did well for all medical handling and procedures Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = eupneic BCS 4/9 EENT: Eyes clear, ears clean, left ear tip, no nasal discharge noted Oral Exam: clean adult dentition with fractured canines and oligodontia, no oral lesions noted PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated, small soft bladder U/G: FS (left ear tipped), no MGTs, no vulvar d/c MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Apparently healthy Plan: Continue to monitor while at BACC Prognosis: Excellent SURGERY: spayed

KNOWN HISTORY:: Sultan was brought in without information on her behavior history or tendencies in a home environment.
MEDICAL BEHAVIOR:: Nervous and will flee if given the chance. Easily handle able. Did well for all medical handling and procedures
ENRICHMENT NOTES:: 01/03/18 Lying in back of kennel, body low and tense. Tolerates petting along head and body while remaining very still, eyes darting around the room. Needs more time to adjust.
Cage Condition:: Cage is neat
Reaction to assessor:: Sultan was tense, wary and crouched on top her kuranda bed with wide eyes focused on the assessor.
Reaction when softly spoken to:: Sultan keeps her eyes hyper focused on the assessor and didn’t get up or come forward when coaxed.
Reaction to cage door opening:: Sultan flinches and lowers her body.
Reaction to touch:: Sultan seems a little unsure of her surroundings and slowly turns her head away when approached by the assessor. She’s tense and first, but slowly starts to relax with petting/attention. Her body arches and she leans in for rubs on her cheeks.

VOCAL:: Quiet

Potential challenges comments:: Sultan has displayed fearful behavior during their stay in the care center and may dislike certain types of handling. A fearful cat will feel more relaxed when given options, so provide her with the chance to move closer, investigate, or interact with you. Be sure to offer incentive such as treats or play time whenever the cat makes a small positive step. Please speak to an adoption counselor for additional information on methods to desensitize your cat to their fear stimulus.
