Medical Summary 03/30/17 08:21 DVM Intake Exam Findings Microchip: negative MC inserted during LVT initial #981020019771406 2yo FI DSH History Surrendered by rDVM (Blue Pearl?) which provided the following: “History: indoor/outdoor cat. no known medical problems, O brought in 3/25/17 after noticing for 3 days cat was not eating and was found hiding in closet and not using left hind limb. brought to CCVC- took xrays and O was not able to be contacted or had called for several days, O finally contacted 3/29/17 and stated unable to pay for surgical repair of fractured limb. O surrendered to CCVC.” Medical records not provided by rDVM Rads emailed to ACC = VD view showing compelte distal physeal transverse non comminuted closed L femoral fracture, lateral view not sent last tx at rDVM 0.13ml Buprenex (0.3mg/ml) SQ and 0.25ml Onsior SQ at 21:00 on 3/29 Subjective Observed Behavior – timid, allowed all handling Evidence of Cruelty seen – no Evidence of Trauma seen – yes Objective T = P = >200 R = 12 BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: no tartar, no ulcerations PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female, spay scar not palpable (abd not shaved to check, reported intact by DVM/o) MSI: toe-touching to nonwt bearing LHL, painful on palpation of LHL, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment closed transverse distal physeal L femoral fracture Plan requested complete records from rDVM 0.13ml Buprenex OTM once during exam 0.16ml Onsior SID SQ x2 days 0.42ml Simbdaol SQ SID x 5days temporary waiver from spay for LHL fx place with NH for primary fx repair px: good with primary fracture repair or amputation
