04/09/17 17:44 Storm is friendly and outgoing towards strangers and usually likes to approach them. She lives with three children, ages 10, 12, and 13, and is relaxed and affectionate with them. She plays exuberantly with both adults and children and has lived with her sister-cat, Storm. They are relaxed and affectionate with each other and usually play both exuberantly and gently. She has never been around dogs so it’s unknown how she would react. Storm has never bitten another animal or human. During car rides, Storm breathes heavily with her mouth open. She struggles when given a bath and loves being brushed. She’s never had her nails trimmed so it’s unknown how she would react to it. She isn’t bothered when picked up or placed inside the carrier. When awoken from her slumber, Storm gets up and starts playing around. Storm is friendly, affectionate, playful, and confident. She has a medium activity level. When her owner is home, she usually follows him around. She doesn’t really play with toys and has been kept indoors only. She sleeps on her owner’s kid’s bed and is litterbox trained. She will only go potty in her hooded litterbox full of clumping litter. Storm scratches on her litterbox or boxes around the house and has never had a scratching post. She eats 9 Lives dry cat food, a bowlful, twice a day. She loves pork chops. On intake, Storm allowed counselor to pet her head and collar her. She stood still for pictures and had a loose body.

Severe amt black granular discharge AU. Pinnal-pedal reflex AS. Eyes clear, no nasal discharge noted. Oral Exam: Moderate tartar, fractured left and right maxillary canine teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female, spayed MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: 1. Ear mites and healed aural hematoma 2. Fractured teeth and dental disease Plan: 1. Cleaned ears, administered 0.15 ml ivermectin SQ. 2. Tresaderm AU BID x5 days 3. Repeat ivermectin in 14 days 4.
