Medical Summary S/O: QAR, MM pink and moist. hypersalivating Oligiodontia, severe dental tartar, right upper canine discolored and loose – swollen gingiva, recessed gingivia, severe gingivitis and halitosis. sensitive to touch. H/L : no obv mumur. lungs clear and eupneic. ABd: soft and not painful. M/S: Amb x 4, with no lameness. palpable spinous process. I: matted haircoat, dander and no ectoparasites A: Severe dental disease thin body condition P: Dental w/extractions Buprenorphine 0.2ml PO BID x 5 days Microchip: negative Sex: neutered male Age: reported 11y Mentation: QARH Eyes: clear Ears: clean Nose: no d/c Teeth: heavy staining and gingivitis (swollen and bleed when touched), malodorous and drooling, some missing teeth If abnormal BCS: overweight Skin: WNL Hair Coat: WNL Declawed: no Any injuries: no Behavior: allows all handling but slighhtly tense, appreciated petting and will approach handler after time Medication: N/A, preventatives given
