EENT: moderate conjunctivitis with mucoid ocular discharge, ears clean, significant mucoid nasal discharge noted
Oral Exam: Pale mm, teeth erupting appropriately
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Female
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: Underage, underweight, URI, conjunctivitis
Plan: Continue to monitor while at BACC. Rec doxycycline 0.15ml PO q24 x 10 days, erythromycin OU q12 x 10 days, nebulize q24 x 3 days. First dose given on medication.
Prognosis: Good

Temporary waiver due to underage, URI
