***SAFE 04/06/17***Snowy is a lovely & friendly 8 year old girl who’s found herself at the shelter and isn’t feeling too well! With plenty of TLC and a warm, loving home to call her own again, this beautiful kitty could live a happy life. Snowy was brought in by owner’s friend who had been caring for her. She currently needs follow up medical care for a laceration on her neck and liver disease.

I came in the shelter as a STRAY on 03/30/2017 from NY 11208, owner surrender reason stated was PET HEALTH.

04/03/2017 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 5.1 LBS.
04/03/17 Monitor condition – liver disease, large wound on neck. S/O: BAR. mm=icteric/slight pink color, CRT<2 s. More social and active today, right at front of cage. All Friskies canned food consumed overnight, also eating some dry food and drinking well. Avoiding special diet. No vomiting/diarrhea. Large amount of urine in litterbox. EENT: Icteric sclera. No oculonasal discharge. H/L: Lungs clear. HR=200, RR=purring. ABD: WNL. MS: Amb x 4. No lameness. INTEG: Skin wound right cervical region as previously described. Area is dry, no bleeding, discharge, swelling, or discomfort. A: Improved appetite, improved hydration, clinically stable cat with liver disease. Large wound on neck static. P: Continue current therapy. Decrease IV fluids to 10 ml/hr, consider discontinuing if she continues to drink well. Avoid SQ route due to cat’s skin fragility. Guarded prognosis. 04/02/17 Monitor condition – liver disease, large wound on neck. S/O: QAR. mm=icteric, tacky. Social and friendly, enjoys attention. No vomiting/diarrhea. Not eating special diet but will eat Friskies. Large amount of urine in litterbox. EENT: No oculonasal discharge. No sneezing. ABD: Soft, non-painful. No masses/organomegaly. MS: Amb x 4, with severe muscle wasting. INTEG: Large, degloving type wound on right ventral cervical area extending to thoracic inlet. SQ tissues and muscle fascia exposed, but there is no bleeding or discharge. Flap of skin is present and partailly covers wound. This area is not painful when gently examine. A: Liver disease, skin fragility (which may be secondary to underlying liver condition). Dehydration approx. 5 %. No signs of deterioration from last exam. P: Restart IV LRS 24 ml/hr. Continue other treatments. Guarded prognosis, especially for wound healing, and is not a candidate at this time for surgical repair of wound. If wound becomes more extensive or cat otherwise worsens, rec. EHR.

03/30/2017 DVM INTAKE PHYSICAL EXAM
Medical rating was 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, behavior rating was NONE
03/30/17 History – Cat was being cared for by owner’s friend who brought her in. History of liver disease, currently on Denamarin 1 tab PO q 24 hours, and Science Diet L/D. No medical paperwork brought from veterinary hospital. Subjective Observed Behavior – Calm, quiet. Allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: Lethargic, AR. mm=icteric, tacky. P = 200 R = 16 BCS = 2/9. ORAL: Icteric mucous membranes. Minimal calculus. EENT: Icteric sclera. Eyes sunken OU. No oculonasal discharge, no sneezing. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. No dyspnea. ABD: Soft, non-painful. No palpable masses/organomegaly. U/G: Female. MS: Amb x 4, weak appearance. Mod. to severe generalized muscle wasting. INTEG: Icteric skin. Dry, somewhat unkempt haircoat. Decreased skin turgor. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Emaciated condition, weakness, dehydration 7-10%, icterus with history of liver disease. DDx: hepatic lipidosis, cholangitis/cholangiohepatitis, toxin exposure, neoplasia. Age difficult to assess due to illness/presenting condition, but does not appear geriatric based on good dental condition and lack of iris atrophy. Est. 8 years. P: Run CBC/Chem/T4. Place IVC, administer LRS at 20 ml/hour. Denamarin feline 1 tab PO q 24 hours. Start Prednisolone 5 mg 1 PO q 24 hours x 7 days (then continue based on reassessment). Start AmoxiClav 45.7 mg/ml. 0.75 ml PO BID x 7 days (continue based on reassessment). Gave first doses of all medications. Feed own food, but if not eating that, offer a variety. Guarded prognosis. If placed, rec. workup with abdominal ultrasound, +/- biopsies, may also benefit from treatment with Ursodiol.
