She’s a sweet girl with a lovely nature and she was given up because her owner was ‘moving’ and of course couldn’t take her. 08/05/16 12:42 Basic Info: Snow Girl A1084232 is a 1yr old female white & gray spotted DSH feline who was given to the owner by the previous owner 2months ago & is being surrendered due to the owner moving to where pets aren’t allowed. Snow Girl hasn’t been seen by a veterinarian and currently has no known health issues/injuries. Socialization: Around strangers Snow Girl is friendly/outgoing. She lived with an 8yr old and was gentle, relaxed, playful & affectionate with them, the adults, a cat and a small dog in the home; although at times Snow Girl was a little too rough with the play biting towards the dog. Snow Girl hasn’t bitten/scratched any person or animal. Behavior: Snow Girl hasn’t been on a car ride, been given a bath, had her nails trimmed or been disturbed while sleeping/resting. She isn’t bothered by being picked up/held or with being placed into a carrier. For a New Family to Know: Snow Girl is described as friendly, playful, affectionate & quiet with a medium activity level. When the owner is home Snow Girl follows them around. She enjoys string, mouse & ball toys and is an indoor only cat that sleeps on the couch. Snow Girl eats dry Meow Mix 2-3x a day. She is uncovered litter box trained with clumping litter within. Snow Girl scratches on the curtains, couch and doesn’t have a scratching post. Intake Behavior: Snow Girl allowed all handling. She allowed to be scanned (-), collared and taken pictures of including positioning her during her photo session. Snow Girl didn’t seem to mind being picked up/held and was placed into a kennel with ease.

Snow Girl’s previous owner states that she is friendly and outgoing around strangers. She has lived with a child and was gentle, relaxed, playful, and affectionate with the child. She got along well with the dog she lived with, but may get a little rough while playing. She was described as friendly, playful, affectionate, and quiet with a medium activity level. Reaction to assessor: Snow Girl comes to the front and was calm, relaxed, and friendly. Reaction when softly spoken to: Snow Girl meows, rubs against the cage door, and reaches out with her paw. Reaction to cage door opening: Snow Girl remains soft and relaxed. Reaction to touch: Snow Girl head-butts, leans in for petting, and begins to purr. Reaction to being picked up: Snow Girl remains calm and purrs when held. Behavior Determination: Beginner Snow Girl interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. She has no known history of behavioral problems. This cat can go to a beginner home.

8/22/16 Poor appetite noted for last few days. This morning, pt had MPND and serosanguinous ocular discharge OD. Extend doxycycline for additional 7 days. Add on Baytril 22.7 mg/ml 0.85 ml SID x7 days and erythromycin OU BID x10 days. Recheck in 5 days. 1088 8/20/16 S/O: record states p didn’t eat, but dry food bowl appears eaten no URI signs P: still rec con’t with meds, monitor app, move out of iso 8/19/16 S/O: tech reports not eating- offered variety of foods A: URI, not eating P: add on mirtazipine 15mg: 1/4T PO every third day for 2 doses, monitor app 8/15/16 has URI, sneezing, nasal discharge seen seen by Dr 1088 , move to isolation and start doxycycline 50mg/ml . GIVE .9cc of doxycycline 50mg/ml PO SID X 10 DAYS-
