Medical Summary DVM Intake Exam Estimated age: 2-5 years based on dentition and general appearance Microchip noted on Intake? Scanned negative History : Ear tipped, finder took the cat into her house for 3 days but she didn’t eat or use the bathroom, could barely walk. Subjective: Purring gently, seems to like being handled. Moves very slowly. Objective T = P = 140 R = purring, eupnic BCS 3/9 Pink MMs, QAR, estimated 8% dehydrated EENT: Clean ears and eyes. Nose slightly crusty but no active discharge. Large scabbed and bleeding lesion on right and left upper lips. Oral Exam: Mild tartar only. Some bloody discharge on lower lips but no active lesions – appears to be just coming from lesion on upper lip PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female, spay tattoo noted MSI: Moves very slowly, weak, generalized muscle wasting. Ambulatory x 4, no lesions noted, skin free of parasites, no masses noted CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: 1. Emaciation, dehydration – R/O seconday to eosinophilic ulcer vs. other underlying disease 2. Eosinophilic ulcer – R/O atopy, food allergy, idiopathic Plan 1. CBC/chemistry running 2. 100 ml SQF administered 3. Tempt to eat Prognosis: Fair-good with appropriate treatment 1088
