Medical Summary DVM Intake Exam Estimated age: 3 months Microchip noted on Intake? none History : 1/6 kittens picked up from owner Uniqua, Doug, Dot, Wakko, Yakko Subjective: Observed Behavior – Ill kitten, difficult to assess Evidence of Cruelty seen – ill kitten Evidence of Trauma seen – 3/6 kittens have one eye w/ abnormal globe Objective T = 101 P = 180 R = wnl BCS 4/9 EENT: OU prolapsed and inflamed nicitans bilat, OD corneal opacity, no nasal discharge noted, AU mild debris, sneezing Oral Exam: nsf PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: nsf MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy , rough coat, underwgt CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: URI OD r/o uri vs trauma otitis underwgt r/o nutritonal, illness, parasites Plan: Terramycin OU then tid x 10 days Doxycycline 50 mg/ml 0.25 ml po sid x 20 days Ear clean vxn when healthy Prognosis: Good SURGERY: Temporary waiver due to URI
