DVM Intake Exam Estimated age: 6weeks Microchip noted on Intake? none History : stray Subjective: nervous but offered canned kitten food and ate with gusto Observed Behavior – allows all handling, docile, nervous Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = 180 R = 24 BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: pink gums deciduous dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FI, grossly normal MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment apparently healthy FI 6 week old Plan ok for adoptions Prognosis: very good SURGERY n/a p not old enough at this time
