SAFE 7/30/18 SETI was brought to the shelter with wounds on the right side of his body. He has a fever and is being treated with antibiotics. SETI needs a home where he can get follow up care.

S: QAR to BARH. Eating well. Moderate amount of soft to formed stool. Moderate urine. Affectionate, allowed all handling.

CRT: <2s. Gums: pink, moist
Eyes: Grossly appropriate OU.
Ears: Unremarkable AU.
Nasal Cavity: No nasal discharge.
Oral Cavity: Mild dental tartar
PLN: WNL
Heart: No murmurs or arrhythmias. Pulses strong and synchronous.
Lungs: Eupneic, lungs clear. No crackles or wheezes bilaterally.
Abd: Soft, non-painful. No palpable masses. No organomegaly appreciated.
U/G: Intact Male. No discharge.
Musculoskeletal: Ambulatory x 4 with no appreciable lameness.
BCS = 5/9
Integument: Three superficial skin wounds at right flank, most dorsal ~3x2cm, mid- flank ~2x2cm, most ventral extending to ventral abdomen ~4x6cm with flap of bruised skin noted, mild purulent discharge noted in all wounds; skin connecting all wounds appears darker (ie slightly red/purple) than rest of skin; area diffusely mild to moderately swollen
Neuro: Appropriate mentation. Full neurologic exam not performed.
Rectal: Not performed. Externally normal.
– The wounds were clipped as much as possible while patient awake (fur around wounds could not be clipped) and area cleaned with chlorhexidine and sterile saline
Assessment:
1) Multiple superficial skin wounds- based on how wounds appear (bruised, swelling), concern for development of SIRS/sepsis
2) Fever- likely secondary to #1
Plan:
Continue Clavamox 13.75 mg/kg PO BID until 8/4
Continue Simbadol 0.24 mg/kg SQ SID until 8/2
Start enrofloxacin 5 mg/kg IM SID until 8/3
Start Onsior 2 mg/kg SQ SID until 7/29
Down for full bloodwork and sedated wound care for tomorrow
