Medical Summary 02/16/17 08:59 S: very sweet, allowed all hanling, seeking attention O: dehydrated EENT: minimal dental tartar, ginvivitis, no oral lesions, Int: crusting lesion on L dorsal neck/ behind L ear, multifocal crusting lesions, alopecia over verntral abdomen and medial R thigh, serosanguinous wound on medial RHL Lnn: WNL CV: NMA, s&s pulses, pale tacky mm Resp: clear lungs, eupnic Abd: SNP UG: intact MS: 3x ambulatory, RHL held in flexion against body, crush injury with wound on medial R metatarsus, severe mm atrophy on R thigh, BCS 4/9 Neuro: BAR, PLRS WNL, meance WNL A: dermatophytosis vs dermatitis vs other crush injury and wound to RHL dehydration P: Woods lamp positive DTM culture taken sedate with DTMB, RHL rads, clip and clean wound, rads: the 2nd digit of RHL completely severed at proximal metatarsal, bony lysis of the severed bones 100ml LRS SQ 2.3ml Clindamycin PO SID x 7d 0.8ml Metacam SQ 0.25ml Buprenex OTM BID x 7d no Lime Sulfur Dips possible dt wound start Terbinafine 125mg PO SID px: good with appropriate treatment, recommend surgery to remove bony sequesterum vs apmutation of RHL Microchip: negative, placed 981020015838686 Sex: intact male Age: appx 4y+ Mentation: BAR, moderate dehydration (tacky mm and prolonged turgor) Eyes: clear Ears: slightly waxy OD, otherwise WNL Nose: no d/c Teeth: mild staining If abnormal BCS: WNL, 4.5/9 Skin: areas of sebaceous activity behind neck near shoulders and small lesions throughout, did not fluoresce under Wood’s, possible old flea derm? Hair Coat: alopecia along abdomen, possibly positional (friction) from injury Declawed: no Any injuries: RHL crush injury over foot, at least three digits involved, purluent d/c from toe “cluster” and most of limb is raw Behavior: allows all handling but painful when hinds are handled, withheld CT due to painful positioning Medication: on call DVM rec. 0.3ml convenia and 0.24ml buprenorphine, fast after midnight for exam in a.m.
