Medical Summary Microchip: positive 4820522337 Sex: spayed female, possible scar felt (hard to tell with skin abnormalities in area) Age: reported 10y Mentation: QARH Eyes: clear Ears: clean Nose: no d/c Teeth: mild to moderate staining If abnormal BCS: WNL, 5/9, muscle wasted along abdomen Skin: WNL Hair Coat: dull, missing hair from LRL foor as if pulled from limb, WNL Declawed: no, polydactyl x4 (6 toes each foot) Any injuries: strips (2-3″) on caudal left side mammaries and midline chest that possibly came from a glue trap (skin is stretched taut with raw areas under substance/scab and debris caught to surface areas), enlarged salivary gland/ln under left side chin Behavior: allowed all handling, generally disinterested in handler but did not resist manipulation Medication: N/A, preventatives given
