Medical Summary 05/17/17 DVM Intake Exam Estimated age: 3 weeks. Microchip noted on Intake? No. History : Stray kitten, noted to have URI. Subjective: Observed Behavior – Quiet, friendly, allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: QARH. mm=lt. pink, moist, CRT<2 s. HR=200, RR=16. BCS=3/9. ORAL: Deciduous teeth erupted. EENT: OU – mucopurulent discharge and crusting. Mild mucoid nasal discharge. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. No dyspnea. ABD: Non painful, no masses/organomegaly palpated. U/G: Male, intact. MS: Amb x 4, no lameness. INTEG: Full, unkempt haircoat. No skin lesions. CNS: Mentation appropriate, no neurologic deficits. A: Underweight kitten with conjunctivitis, URI. P: Offered food – started eating immediately. Rx Erythromycin ointment Apply OU BID x 10 days. Rx AmoxiClav 45.7 mg/ml. 0.1 ml PO BID x 10 days. Good prognosis. Temporary waiver from neuter surgery due to being underage, underweight.
