Medical Summary Scanned positive with Chip #4260711522 QAR – Began exam with head butting and rubbing; quickly became agitated and began growling and tried to scratch. Exam completed in towel. F~ 10 years. Attempted to shave for spay scar but was unable to restrain. Palpates normal within towel. Ears, nose, throat clear Mild to moderate dehydration. Emaciated – good appetite when fed. Loose yellow-brown stool found in cage – tests negative for panleukopenia. Eyes – slightly sunken, yellow discharge from left eye. Teeth deeply stained, worn, grooved. Mild to moderate gingivitis and tarter. Halitosis. Coat dirty. Lightly matted across body. Does not tolerate grooming or shaving. Flea dirt found. No live parasites noted Ambx4 Nails long – unable to trim. NOSF
