SAFE 4/22/17 Sam is cute long-haired 3 week old kitten who is underweight and needs some TLC. Needs placement ASAP!

Medical::
04/20/17
DVM exam.
History – Stray kitten, no other history known.
Microchip scan negative.
Age est 3 weeks.
Observed behavior – Quiet, becomes active and vocal when picked up. Allows all handling.
Evidence of cruelty – No.
Evidence of trauma – No.
O: QARH. mm= lt pink, moist, CRT<2 s. HR>240, RR=20. BCS=2/9.
ORAL: Deciduous canines and incisors have erupted. Premolars starting to erupt. No oral lesions.
EENT: No oculonasal discharge. Ears WNL AU.
H/L: Lungs clear, no murmurs/arrhythmias.
ABD: Soft, non-painful. No palpable masses/organomegaly.
UG: Male.
MS: Amb x 4, appears a little wobbly but no tremors noted.
NEURO: Mentation appropriate, no neuro deficits.
INTEG: Full, clean haircoat. No skin lesions. Mildly decreased skin turgor.
PLN: No peripheral lymph node enlargement.
A: Approx 3 weeks old, underweight kitten. Mildly dehydrated.
P: Offered kitten food – started eating immediately.
10 ml LRS SQ administered.
House in medical to monitor appetite/overall condition.
Temporary waiver from neuter surgery due to underage/underweight
Rec. placement.
Good prognosis.
04/21/17:
Monitor condition, underage/underweight kitten slightly dehydrated on intake
S/O: QARH, becomes vocal when handled, able to eat on own
EENT: Eyes clear, small amount of crusted discharge at medial canthus, no nasal discharge
HL: Normal thoracic auscultation
ABD: Normal abdominal palpation
INTEG: Full coat, no signs of ectoparasites or alopecia
MS: Ambulatory x 4
UG: Male
A: Underage, underweight
P: Rec foster/NH placement. Good prognosis
