*** SAFE 10/15/17 *** DIABETIC – 15 year old Salem was brought to us due to landlord issues and personal problems. He is a sweet boy and timid at the moment in his cage. He is currently on insulin for Diabetes.

Hello, my name is Salem. My animal id is #9447. I am a male black cat at the Staten Island Animal Care Center. The shelter thinks i am about 15 years old. – P
I came into the shelter as a owner surrender on 11-Oct-2017, with the surrender reason stated as person circumstance- landlord won’t allow.

Scan negative BAR, moderate dehydration AMBx4 Nervous/tense, but allowed handling Male~15yrs Teeth: mild gingivitis/tartar Eyes: slightly cloudy OS Ears: Debris seen, ear mites(-), cleaned Nose/skin: WNL Heart: possible heart murmur , will confirm on DVM intake. Lungs: WNL As per owner: frequent urination High glucose amount in urine and on CBC As per Dr. 0844 administered 100ml LRS administer 1Unit of PRoZinc insulin, if eating. Will monitor appetite and check BG later in day.

[DVM Intake] DVM Intake Exam Estimated age:15 years History : owner surrender Subjective: Observed Behavior – friendly Evidence of Cruelty seen -no Evidence of Trauma seen -no Objective BCS 3/9 EENT: Eyes clear, AU-mild brown waxy debris, no nasal or ocular discharge noted Oral Exam: moderate dental calc and gingivitis PLN: No enlargements noted H/L: NSR, grade 3/6 heart murmur, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI, 2 descended testicles MSI: Seems weak in hindlimbs, skin free of parasites, no masses noted, dorsal muscle wasting; dry scale on haircoat CNS: Mentation appropriate Assessment severe hyperglycemia and glucosuria- R/O DM vs other urine dipstick: leukocytes 3+- R/O UTI vs other; glucose 4+, negative ketones underweight moderate dental disease heart murmur Prognosis: guarded Plan: BG this morning= 641 eating well, feed only canned diabetic food or canned kitten food continue PZI insulin 1 unit every 12 hours for now and monitor BG and ketones gave convenia 0.4 mls SQ recommend placement as soon as possible for further diagnostics and treatment SURGERY: Permanent waiver at ACC due to diabetes, age, and heart murmur

Behavior around children: Either doesn’t play or plays gently. Behavior around dogs: Watches from a distance and swats if they try to play with him. Bite history: Salem has never bitten a person or another animal. Energy level/descriptors: Medium activity level Other notes: Owner described Salem as friendly, affectionate, and independent. When they were home Salem would follow them around making sure to be in the same room or be in a favorite spot. Salem likes balls and wand toys. Salem was kept indoors only. MEDICAL BEHAVIOR: Observed Behavior – friendly Cage Condition: Cage is neat Reaction to assessor: Salem was lying down by the back, calm and relaxed. Reaction when softly spoken to: Salem looks at the assessor with dilated eyes, and then averts his gaze. Reaction to cage door opening: Salem remains motionless. Reaction to touch: Salem remains in place and allows gentle petting on his head and body. His ears tilt slightly to the side, but he doesn’t seem to mind attention. Reaction to being picked up: Did not attempt due to medical condition. ACTIVITY LEVEL: Mellow VOCAL: Quiet CHARACTER TYPE: Shy Calm Sweet POTENTIAL CHALLENGES: Inappropriate elimination Potential challenges comments: Owner stated that Salem has been urinating on the carpet about every other night. We are unaware of what, if any, modification steps were taken to try and address this behavior. Please note as well that Salem has been diagnosed with and is being treated for diabetes in the care center, which may be influencing the urination behavior. We cannot be sure whether this behavior will continue in a home environment after treatment for the medical condition has been completed, but we recommend that potential adopters be comfortable with management and modification techniques for elimination outside the litter box should this behavior continue in a home environment. BEHAVIOR DETERMINATION: Average Behavior Asilomar H – Healthy BEHAVIOR SUMMARY: Please note that this cat is being treated for a medical condition at the time of evaluation. It is difficult to determine at this time how the medical condition may be affecting the behavior. Salem interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.
