Medical Summary DVM Intake Exam Estimated age: 8months Microchip noted on Intake? none History : Subjective: meowing and scratching adamantly at cage doors to get out Observed Behavior – once out of cage, purring and likes to be pet Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective unable to ascult because purring, but eupneic BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mild gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FI, clipped fur, didn’t palpate any spay scar MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, both FLs nails are slightly bloodied, no active bleeding or open wounds now- from scratching cage doors CNS: mentation appropriate – no signs of neurologic abnormalities Assessment apparently healthy FI FL nails slightly bloodied from scratching at cage doors Plan ok for spay offered food- ate well scheduled for foot soaking for nails Prognosis: good SURGERY: Okay for surgery
