DVM Intake Exam Estimated age: 5 years Microchip noted on Intake? neg History : found in an apartment building Subjective: BAR Observed Behavior – somewhat unsocialized, very fractious, swatting, growling, lunging; used thick gloves to handle but growled and vocalized for the entire exam and tech check Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 7/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pk, moist; CRT <2 sec; mild to moderate tartar; limited oral exam but there is an ulcerated area of inflammation cranial to L upper premolar, possible resorptive lesion PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: abdomen appears distended as if possible pregnant but is soft and nonpainful with no palpable abnormalities U/G: female; no spay scar or tattoo noted; no vulvular discharge noted MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Obesity Gingival ulceration Plan Rec’d weight loss through dietary management Rec’d dental cleaning Prognosis: good SURGERY: Okay for surgery
