Medical Summary DVM Intake Exam Estimated age: approx 5-6 weeks of age Microchip noted on Intake? No History : P presents as stray. BAR- great appetite. Friendly and allows all physical handling. Large amount of light tan diarrhea leaking from rectum. Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective P = 220 R = 50 BCS: 3/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Decidious dentition, MM pink and moist PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Large distended abdomen, Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted, Unkempt hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Leaking tan soft diarrhea, large amount of fecal staining around rectum. Assessment: Diarrhea with distended abdomen- r/o intestinal parasites vs FIP vs other Underage Underweight Plan: LRS SQ 20 ml, Deworm- continue to monitor diarrhea Parvo SNAP- negative Prognosis: Good to fair depending on underlying cause SURGERY: Temporary waiver due to underage, underweight
