DVM Intake Exam Estimated age: 8 months Observed Behavior – docle, depressed Objective depressed dehydrated P = WNL R =stertorous breathing BCS 5/9 EENT: Eyes: right eye crusted shut with blood, left eye crusted ocular discharge, entire face and nostrils crusted with blood. Clear corneal surface OU. clean AU Oral Exam: open mouth breathing, dried blood on hard palate, dentition, exposed bony surface of mandible due to degloving injury affects entire chin and skin on mandible PLN: mandibular esp left side enlarged H/L: refered upper airway sounds ABD: Non painful, no masses palpated U/G:scrotal testes MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, no fleas ,litter attached to dried blood on face CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment facial trauma – degloving injury to chin and all of mandibular skin. Malodor Prognosis: guarded Plan: alert NH about case and 24 hr deadline flushed face gently with eyewash and soft gauze- continue TID convenia 0.28ml SC Simbadol 0.42ml SC cont SID IV catheter 22 gauge right cephalic combo test neg pet cannot eat on own, needs a feeding tube placed and facial surgery
