Medical Summary DVM Intake Exam Findings History: Hold confidential – owner was evicted Subjective Observed Behavior – relaxed posture, inquisitive, allows all handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = wnl R = wnl BCS 3/5 (5/9) EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Grade 2/4 dental dz PLN: No enlargements noted H/L: Grade III-IV/VI heart murmur Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: wnl MSI: Ambulatory x 4, skin free of parasites, no masses noted, alopecia caudo-ventral abdomen, mild miliary dermatitis CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment – Alopecia with mild miliary dermatitis (no signs of external parasites) DDx: Food allergy, Atopy, Open; Heart murmur; Medical Behavior Status GREEN Medical Status – 3NC Asilomar Status – TREATMANAG Fast Track? No Plan Already neutered. Recommend rescue placement or adoption to someone willing to take on multiple medical conditions. Should have heart murmur worked up including, but not limited to CardioPet proBNP, chest rads +/- echocardiogram. Recommend food trial and/or referral to veterinary dermatologist for skin issues. Recommend dental prophy.
