I came in the shelter as a STRAY on 06/24/2017 from NY 10019, owner surrender reason stated was STRAY.

DVM Intake Exam Estimated age: 5-7 years Microchip noted on Intake? placed on intake 981020019632933 History : brought in as a stray Subjective: BAR Observed Behavior – hissing, allows handling initially but started resisting restraint quickly Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 5/9 EENT: eyes/nose clear AU-ears cleaned during LVT intake but residual dark discharge remains; pruritic Oral Exam: mm pk, moist; CRT <2 sec; mod tartar/staining PLN: No enlargements noted H/L: grade IV/VI L heart murmur ABD: Non painful, no masses palpated U/G: male intact; 2 scrotal testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Ear mites Heart murmur Plan Ivermectin 0.15 ml SQ, repeat in 2 weeks Rec’d full cardiac work up ideally with echocardiogram; at minimum, rec’d CXR, BP monitoring and ECG Prognosis: fair; concern for primary heart disease due to severity of murmur SURGERY: permanent waiver d/t heart murmur

06/24/17 20:54 During intake Roosevelt allowed all handling and collaring. He also allowed all handling with his Finder. He was rolling around inside the carrier when the counselor was petting him.

ACTIVITY LEVEL: Laid back VOCAL: Quiet CHARACTER TYPE: Sweet, Affectionate EVALUATION: Cage Condition: No change Reaction to assessor: Roosevelt immediately comes soliciting at the front of the cage. Reaction to door opening: Rooselvelt remains standing at the front of the cage, relaxed body posture, soliciting attention. Reaction to touch: Roosevelt head-butts the assessor’s hand and appreciates petting on the head and body. Reaction to being picked up: Roosevelt allows the pickup and remains calm. BEHAVIOR SUMMARY: Beginner Roosevelt interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. This cat can go to a beginner home.
