DVM Intake Exam Estimated age: 1 year microchip scan: positive History : owner surrender, history of corneal ulcer, heart murmur, and inappropriate urination Subjective: Observed Behavior – very vocal, resisted exam Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective BCS 5/9 EENT: OD- conjunctival hyperemia and scleral injection, corneal opacity, ears clean, no nasal discharge noted Oral Exam: clean adult teeth PLN: No enlargements noted H/L: grade 4/6 heart murmur, regular rhythm, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI, 2 descended testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment heart murmur hx of inappropriate urination- R/O spraying vs medical vs other corneal ulcer OD- appears to be healing Plan fluoroscein stain OD: no stain pick up erythromycin ophthalmic ointment OD BID x 7 days Prognosis: fair SURGERY: Permanent waiver at ACC due to heart murmur recommend an echo with a cardiologist first and then if okay for anesthesia can go ahead with neuter

05/21/17 17:11 Basic Information: Rio is a year old male Gray Tabby & White DSH that has a heart murmur and ulcer in right eye. Previous owner has had Rio since November 2016 when she adopted him for SIACC but is returning him due to his heart issues. Socialization: According to the owner, around strangers, Rio is friendly and outgoing. Around adults & children (4 & 10), Rio is respectful and relaxed and plays in a gentle manner. He has never been around cats so his behavior is unknown. Around a large size dog (Husky mix), Rio is very tolerant and playful but becomes territorial over his food. He has never bitten a person or another animal before. Behavior: Previous owners stated that Rio does not have any behavioral issues. Rio does spray constantly in the corner of previous owner’s room. Owner did not bath or trim Rio’s nails so it is unknown how he would react to these grooming agents. Rio enjoys by having his coat brushed. Rio allows being picked up and held but only for a short period of time, he will attempt to struggle free when he has had enough. Rio isn’t bothered by being put into a carrier and he meows during car ride. For a New Family to Know: Rio is described as friendly, affectionate, and playful with a medium activity level. When in the home, Rio is following you around wanting attention. He loves to play with any and all toys. She was kept indoors and is litter box trained. Rio used an uncovered litter box and non- clumping litter. He was fed dry cat food and cat wet food. Rio did not have scratching post but he does not really scratch.

Indoor Previously lived with: adults and children Behavior toward children: Around adults & children (4 & 10), Rio is respectful and relaxed and plays in a gentle manner. Behavior toward strangers: Rio is friendly and outgoing. Litter box training: yes but previous owner did report Rio spraying in corners. Energy level/descriptors: Rio is described as friendly, affectionate, and playful. EVALUATION: Cage Condition: recently cleaned Reaction to assessor: Rio is in the front, soft, calm. Reaction when softly spoken to: Rio does not respond, body soft. Reaction to cage door opening: Rio meows and stand up. Reaction to touch: Sparky leans into petting, vocalizing and raises his paw, solicits petting Reaction to being picked up: Rio was tolerant but quickly jumped back. MEDICAL BEHAVIOR: very vocal, resisted exam. ACTIVITY LEVEL: Medium VOCAL: Talkative, meows. CHARACTER TYPE: friendly, affectionate, playful. RECOMMENDATIONS: _x_ Experienced cat parent POTENTIAL CHALLENGES: _x_ inappropriate elimination Rio’s previous owner reports that he was praying in corners around the house, but we are unaware of what, if any, modification steps were taken to try and address this behavior. Please note as well that Rio has been diagnosed with a heart Murmur and is being treated for an eye ulcer in the care center, which may be influencing the urination behavior. We cannot be sure whether this behavior will continue in a home environment after treatment for the medical condition has been completed, but we recommend that potential adopters be comfortable with management and modification techniques for elimination outside the litter box should this behavior continue in a home environment.
