Medical Summary DVM Intake Exam Estimated age: reported to be 7 by previous owner; appears as if she could be younger (3-5 years) Microchip noted on Intake? yes #985111000317154 History : owner surrender due to owner’s housing issues Subjective: BAR Observed Behavior – friendly and curious initially but started growling during oral exam Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 6/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pk, moist; CRT <2 sec; fracture of upper R canine with root exposure; mod gingivitis, tartar and staining PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: soft, nonpainful, sl doughy; no palpable abnormalities U/G: spayed MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment Canine tip fracture with root exposure Gingivitis Plan rec’d dental cleaning with likely extraction of upper R canine Prognosis: excellent SURGERY: spayed
