Medical Summary DVM Intake Exam Estimated age: 5 weeks Microchip noted on Intake? neg, too young to place History : removed from a hoarding situation Subjective: QAR Observed Behavior – mews at front of carrier for attention, allows handling; good appetite with canned food mixed with water and KMR Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 4/9 EENT: OU-elevated 3rd eyelids, mild blepharospasm Oral Exam: mm lgt pk, tacky; all deciduous dentitia present PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: soft, nonpainful; sl doughy; intestines feel slightly fluid filled; noted to have diarrhea U/G: female MSI: ambulatory x 4 with no obvious lameness or injury fleas/flea debris CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: mild irritation secondary to diarrhea Assessment Diarrhea Underage/underweight Conjunctivitis Plan parvo test-neg erythryomycin OU BID x 7 days ponazuril 0.5 ml PO SID x 3 days Prognosis: good SURGERY: temp waiver due to underage/underweight
