Medical Summary DVM Intake Exam Estimated age: 4 weeks Microchip noted on Intake? n History : stray Subjective: patient is BAR, underweight however when food was offered patient ate very well Observed Behavior – allowed all handling Evidence of Cruelty seen – n Evidence of Trauma seen – n n Objective P = 200 R = 28 BCS 3/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: wnl PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: testicles palpable, difficult to feel two MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment – underweight Plan – monitor for signs for URI Prognosis: good Temporary waiver due to age, size.
