Medical Summary DVM Intake Exam Estimated age: 2 mo Microchip noted on Intake: Negative History: Stray Subjective: Observed Behavior – BAR Evidence of Cruelty seen – None Evidence of Trauma seen – None Objective: Hydrated; T =NT P = 170 R = 28 BCS 5/9 EENT: Eyes clear, ears clean, thick green nasal discharge noted Oral Exam: No dental tartar, all deciduous teeth are present, MM Pink and moist, PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities (full neurologic exam not performed) Rectal: Not performed Assessment 1. URI Plan/Treatment/Recommendations 1. Clavamox 0.25 ml PO BID 10 days 2. Nebulize BID 7 days Prognosis: Good once treated for URI SURGERY: Temporary waiver due to URI

BAR. Not eating food in cage. No c/s/v. Sneezing and congested
EENT: Eyes clear thick yellow nasal discharge noted
H/L: Sneezing and very congested
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
Continue Clavamox 0.25 ml PO BID
Continue Nebulize BID
Start doxycycline 10mg/kg PO SID
Add syringe feeding-if still not eating add mirtazapine tomorrow
Prognosis: Good once treated for URI
